Add Labels to Questionnaire

For Claim Questionnaire Select Claim Types from the Admin menu

Choose a Claim Type and click on the Edit button

Open the Questions tab

Add a Label

1) Add new question

2) Select question type drop down and choose Label

3) Enter Label text

The label item is displayed as text in the questionnaire on the client window
